Documents Needed

Once your account has been created, you will be assigned tasks to upload documents to verify your identity and credentials.

Please be prepared to upload the following documents:

• License or certification (example: RN license or CNA certification)

• Tax identification number (SSN or EIN) for IRS Form W-9

• Government-issued photo ID (driver's license, passport, military ID, etc.)

• CPR/BLS card (completed within the past two years)

• TB test results (completed within the past 12 months)

• COVID-19 vaccination card (optional)

Depending on your location, specialty, or the facility where you intend to offer services, additional credentials may be requested. Any additional required or optional credentials will be assigned as a task in your app.
