Accepted Forms of ID

CareEvo must verify your identity before you can be given access to claim shifts. When you are assigned the task to upload a photo ID, please upload a government-issued document that clearly shows your full name and date of birth.

Accepted documents include:

• Driver’s license

• Passport

• Military ID

• Other official government-issued identification
